The Community Health Nurse conducts comprehensive assessments and coordinates personalized care plans for clients with complex, chronic health needs in home and clinic settings. They collaborate with an interprofessional team to facilitate transitions across the healthcare continuum and provide direct clinical care to support client independence. Candidates must hold current practicing registration as a Registered Nurse with the BCCNM and possess at least one year of recent clinical experience in acute or community settings. A valid BC driver's license and access to a personal vehicle are also required for this role.
