The Senior Social Worker is an experienced clinician who provides short-term case management support to patients in acute care and urgent care areas . The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to assess and support patients and/or to coach, mentor, orientate and provide education to an assigned team in DPAS. Drawing on standards of practice for case management; professional codes of ethics; social work standards of practice in health care; program policy and procedures; and relevant legislation, the Senior Social Worker provides high quality patient service.
