We are seeking a dedicated and relationship-focused Client Care Coordinator to join our team in Vancouver. This role is ideal for someone who combines strong client service and communication skills with a genuine passion for supporting older adults and their families. As a Client Care Coordinator, you will play an important role in ensuring our clients receive the highest quality of care. You will work closely with clients, families, CAREGivers, healthcare professionals, and community partners to assess needs, develop individualized care plans, coordinate services, and maintain strong ongoing relationships. This is a full-time position offering $60,000 annually, plus benefits, with opportunities for professional growth in a collaborative and supportive work environment. Primary Responsibilities Conduct client and family consultations to understand care needs, preferences, and goals. Complete client assessments and develop individualized care plans. Introduce and match CAREGivers with clients based on care needs, preferences, and compatibility. Conduct regular quality assurance visits and follow up with clients and families to ensure satisfaction and quality of care. Monitor and update care plans as client needs change. Maintain accurate and timely client documentation, assessments, care plans, and reports. Build and maintain strong working relationships with clients, families, CAREGivers, healthcare professionals, and community partners. Collaborate with medical professionals and other service providers involved in a client's care. Respond promptly and professionally to client and family concerns and coordinate appropriate follow-up. Support the development of strong community relationships and promote Home Instead services to healthcare and community organizations. Organize, prioritize, and coordinate multiple responsibilities in a fast-paced environment. Communicate effectively with individuals at all levels, both verbally and in writing. Work independently while contributing actively to a collaborative team environment. Participate in presentations, community outreach, and relationship-building activities as required. What You Bring to the Role We are looking for someone who is compassionate, organized, proactive, and confident working with older adults and their families. The Ideal Candidate Will Have Excellent verbal and written communication skills. Strong interpersonal and relationship-building skills. Sound judgment and the ability to make thoughtful decisions. Excellent organization, prioritization, and time-management skills. The ability to work independently, take initiative, and remain accountable for responsibilities. A collaborative, team-oriented approach. Strong problem-solving skills and the ability to manage competing priorities. Comfort using technology and various computer systems. Professional presentation and demeanor. Confidence communicating with clients, families, healthcare professionals, and community partners. A valid driver's license and reliable access to transportation. Experience & Qualifications Experience or knowledge in the following areas is considered an asset: Home care, healthcare, social services, or senior care. Working with older adults and their families. Alzheimer's disease and other forms of dementia. Client assessments, care planning, or case management. Nursing, social work, or another healthcare-related discipline. Customer service, sales, relationship management, or business development. A background in nursing (LPN), social work, healthcare, home care, or a related field is strongly preferred. Equivalent combinations of education and relevant professional experience will also be considered.
