As a Physician Assistant or Nurse Practitioner, you’ll join a purpose-driven team that: Delivers continuum-based care – integrating primary, psych, palliative, and home-based services. What Makes this Role Unique Continuum-Based Care (Clinical Excellence): Deliver high-quality care across home, senior living, and community settings, supported by a multidisciplinary team in palliative, psych, and neurology. Dementia Leadership: Build deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative. Your Responsibilities Include: Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities. Delivering dementia-informed, relationship-based care across the continuum of care. Conducting comprehensive assessments and developing tailored care plans for older adults. Managing chronic conditions and performing care with skill and empathy for patients and their families. Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care. Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals. Maintaining consistency in documentation, follow-through, and patient communication. Compensation Competitive Pay: Guaranteed Base Income + Productivity (dependent on number of days worked). Flexible Schedule: Enjoy stability and balance while making a measurable difference in patients’ lives. What You’ll Bring Active, unencumbered NP or PA license in the state you will be practicing. Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred). Compassionate, patient-centered approach and strong clinical judgment. Valid driver’s license and reliable transportation.
