Mara Home & Health Services Inc. is seeking an experienced Nurse Practitioner for a proposed primary care assignment supporting young people at Justice Ronald Lester Youth Centre in Thunder Bay, Ontario. This in-person opportunity combines autonomous clinical practice with collaboration across a multidisciplinary team. You will provide compassionate, youth-centred care to young people in custody, including those with complex physical health, mental health, substance use and trauma-related needs. With two scheduled daytime clinics each week, this assignment may complement an existing clinical practice while offering the opportunity to build continuity of care and make a meaningful contribution to youth health. Meaningful clinical work: Help young people access assessment, treatment, education and coordinated follow-up. Professional autonomy and collaboration: Practise within your NP scope while working with healthcare professionals and facility staff. Onsite clinical resources: Clinical space, examination equipment and primary care supplies are provided. Potential for continuity: The proposed service contract has an initial two-year term, with up to three additional one-year extensions at the client’s discretion. In this role, you will: Complete comprehensive intake health assessments and provide ongoing primary care. Assess, diagnose and manage acute illnesses, injuries, chronic conditions and communicable diseases. Prescribe medications and order, interpret and follow up on laboratory tests and diagnostic investigations within your authorized scope. Assess physical and mental health concerns, arrange referrals and escalate urgent issues when appropriate. Provide age-appropriate health education, counselling and preventive care. Maintain timely clinical documentation, assessment summaries and discharge reports. Collaborate with facility staff and healthcare providers to support coordinated care. Follow infection prevention, privacy, consent and facility security requirements. Provide or coordinate reproductive and other gender-specific healthcare within your competence as the needs of the youth population evolve. Applicants must have: Active College of Nurses of Ontario registration in the Extended Class, in good standing and authorized for autonomous practice, with no active disciplinary proceedings. Current Ontario prescribing authority, including controlled substances where applicable. Professional liability insurance meeting the assignment’s requirements. Experience providing care to children, youth or young adults. Experience with mental health, substance use and concurrent disorders. Experience in forensic, correctional or institutional settings, or other complex care environments serving marginalized populations. Demonstrated experience delivering trauma-informed and culturally safe care, including care for Indigenous and racialized young people. Experience in communicable disease management and infection prevention and control. Strong clinical judgment, documentation, communication and teamwork skills. Legal authorization to work in Ontario. Availability to attend both weekly onsite clinics and complete required orientation, security screening and health and safety training. The schedule may be adjusted to meet facility needs, and occasional rescheduling within the same week may be necessary. Candidates should also be willing to support reasonable requests for urgent telephone or approved virtual clinical consultation outside scheduled clinic hours.
