The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care, and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home. Potential Sign on Bonus of $9000. You will be part of a diverse care team that will provide services to a roster of clients in the community. You will be part of a collaborative team that will conduct joint care planning with a focus on client-centered goals, addressing the client’s physical, cognitive, social, emotional, and spiritual domains, and creating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate the interdisciplinary team to reach the client’s care goals. Job Responsibilities Conducting assessments Formulating goals and recommendations Planning and implementing client-centered, goal-directed treatment programs Requirements A graduate of an accredited program in Occupational Therapy and are a member in good standing with the College of Occupational Therapists of Ontario Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team Sound knowledge of community resources is an asset Effective interpersonal, organizational and communication skills A positive work and attendance record is essential Ability to adapt to new computer applications This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting A vehicle and valid driver’s license and the ability to travel About SE Health SE Health is a not-for-profit social enterprise advancing health with heart. Requirements
