Position Summary: * Responsible for development of nutrition care plans and counselling for patients with diabetes. * Use a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change, i.e. MI, stages of change * Broad understanding of nutrition resources available in the community to support clients with complex needs * Functions as part of an inter-disciplinary team and acts as a resource for other health care providers * Facilitate group education programs at the DMC * Develops nutrition education resources * Participates in program planning and evaluation Key Qualifications: * Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program * Member in good standing with the College of Dietitians of Ontario * Current membership in Dietitians of Canada and Diabetes Educator Sector of Diabetes Canada preferred * Certified Diabetes Educator preferred * At least 3 years current experience working with clients with diabetes in a health care setting. * Good working knowledge of diabetes and other comorbidities through the lifecycle and adult teaching principles * Individual counselling and group counselling experience preferred * Demonstrated leadership and communication skills * Positive work and attendance record essential * Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment * A second language an asset Internal Candidates who believe they possess the necessary qualifications and experience for this position and who have been in their current position for at least six (6) months are encouraged to apply. Requirements
Candidates must hold a degree in Human Nutrition and Dietetics and be a member in good standing with the College of Dietitians of Ontario. At least 3 years of current experience working with clients with diabetes in a health care setting is required.
