Job Description Reporting to the Director, Cardiovascular, Chronic Disease and Stroke, the Nurse Practitioner (NP) will be responsible for the provision of clinical care within the Stroke program. Accountabilities: Responsible for the comprehensive assessment of patients with stroke/TIA diagnoses for both the outpatient stroke prevention clinic,and inpatient stroke unit admissions with follow up as appropriate Initiate treatment in collaboration with the physician, including health management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits and conditions, and employer policies and procedures Provide professional guidance to other health care professional and participates autonomously as a member of the interdisciplinary health team Initiate referrals to specialist physicians as appropriate Collaborate with clients, health care professionals and stroke neurologists to identify and assess trends and patterns that have implications for clients, families and communities Develop and implement population and evidence based strategies to improve health, and participate in policy making activities that influence health services and practices Responsible for personal professional development, and maintaining competence in the position Qualifications RN(EC) in good standing with the College of Nurses of Ontario Membership in professional organizations (NPAO, RNAO) an asset Recent Experience (within the last 2 years) with adult populations with stroke/chronic diseases is required Specialty certificate in neurology/stroke preferred but achievement of the Hemispheres certificate is required within 3 months of hire Minimum two years of recent relevant clinical nursing experience on stroke/neurology Neurology/stoke experience or critical care background preferred Core competencies: Demonstrated assessment and critical thinking skills as they relate to stroke patient management Knowledge of computerized data management and statistical analysis Ability to perform the full scope of duties and responsibilities of the NP role as an independent practitioner while working under a Collaborative Practice Agreement Ability to apply knowledge of pharmacology, pharmacokinetics and pharmacodynamics to prescribing and evaluating treatment responses Ability to communicate and collaborate with patients and families about health findings, diagnosis, treatment, self-care and prognosis Ability to collaborate, consult with and formally refer clients and patients to physicians and other health professionals when appropriate Ability to critically assess and evaluate health research literature to determine best practices; ability to introduce education and evidence-based research Ability to assess and recognize population health trends; ability to plan and implement strategies for populations based prevention and health promotion Ability to implement and evaluate planned change Ability to define the specific areas of practice and client population for whom the position is competent to provide health care services Ability to self-direct, interact and adapt effectively with other professionals in complex, dynamic situations Ability to transfer knowledge, coach and mentor others Ability to identify and respond appropriately to legal and ethical issues that may arise in patient care Ability to self-assess performance and assume responsibilities and accountability for own professional development, educational or consultative assistance when appropriate Additional Information Hours: Currently days, 8 hour shifts, including weekends and statutory holidays (subject to change in accordance with operational requirements) SALARY: Minimum: $65.46 Maximum: $81.53 Internal application deadline: Tuesday, September 15, 2026 Applications are still open for external applicants HOOPP Pension Plan: Secure Your Future with Osler When you join Osler, you're not just starting a job; you're investing in your future. Requirements

Candidates must hold an RN(EC) designation in good standing with the College of Nurses of Ontario and possess at least two years of recent clinical experience in stroke or neurology. A specialty certificate in neurology or stroke is preferred, and the Hemispheres certificate is required within three months of hire.
