JOB SUMMARY: As Nutrition Coach you will support the diet and nutrition needs of the people we support as they work towards their goals and plans for life redesign. Always coming from a place of ‘eating as a pleasurable experience’ you will meet with individuals soon after they move in and periodically throughout their stay with us to determine their diet and nutrition needs and goals, and to guide and inform service delivery. CONNECT homes are staffed by Life Redesign Coaches. On hand 24/7 they are people with diverse backgrounds whose collective strengths offer a rich resource to the people we support. You must be comfortable working with and through them, and willing to coach, mentor, or teach when a need arises. REPORTS TO: Leader of Culture, People and Services START DATE: September JOB REQUIREMENTS & COMPETENCIES: * A Registered Dietitian in good standing with the College of Dietitians of British Columbia (for CONNECT BC locations) or the College of Dietitians of Ontario (for CONNECT Ontario locations), and possesses/demonstrates: * Experience supporting individuals with acquired brain injury, stroke, neurological conditions, and other complex health needs; * Skill in coaching and mentoring others—translating nutrition knowledge into everyday, accessible strategies for non-clinical team members and families; * Commitment to person-centred, strength-based, and relationship-focused practice; and, * Flexibility, creativity, and a willingness to work outside traditional healthcare models. CONNECT homes are staffed by Life Redesign Coaches—individuals with diverse backgrounds whose collective strengths support residents in their journey. You must be comfortable working both with and through others, supporting and guiding them in their role, and being available as part of a collaborative support network when needed. KEY TASKS & RESPONSIBILITIES: * Understand each person’s story, values, and goals, and how food and nutrition support their life redesign * Assess nutrition needs, eating habits, and preferences in real-life settings * Develop and contribute ideas and strategies that are meaningful, practical, and aligned with individual goals * Ensure all strategies are developed collaboratively and respectfully with the individual and their support network * Support medically indicated diets in ways that maintain dignity, autonomy, and enjoyment of food * Create and support positive, inclusive, and engaging mealtime experiences * Show others how to integrate nutrition strategies into daily routines * Make use of real-life opportunities for learning and growth * Collaborate with team members to ensure one integrated Life Redesign Plan * Maintain appropriate documentation of involvement * Manage time and responsibilities effectively to meet role expectations * Provide updates and recommendations for quality improvement to the Leade WORKING CONDITIONS * Part time 8 hours a week PERFORMANCE DEVELOPMENT & REVIEW CONNECT offers a program of continuous learning activities as well as funding for conferences, workshops and courses. Candidates must be a Registered Dietitian in good standing with the relevant provincial college. Experience in supporting individuals with complex health needs, such as brain injuries or strokes, is required.
