The RN Care Coordinator provides wholistic home care nursing services and manages caseloads to support the wellbeing and independence of Indigenous clients. This includes developing individualized care plans, coordinating interdisciplinary services, and facilitating seamless transitions between hospital and home. Candidates must be a Registered Nurse in good standing with the BCCNM, hold a Bachelor's Degree in Nursing, and have 3-5 years of relevant experience. Additional requirements include a valid BC Driver's License, BLS certification, and a clear criminal record check.
